The patient is a 42‑year‑old unmarried male, presented by his father with complaints of irritability, suspiciousness, self‑muttering, restlessness, poor hygiene, and irrelevant talk persisting for the past two years. He was functioning well until 2023 when, while working as an engineer, he had a conflict with junior workers, after which he developed sleep difficulties and gradually became irritable and suspicious. His father reported frequent job changes and inability to sustain employment, as he would leave companies without completing interviews. At home, he was often seen hurling abuses, which the family later realized were directed at imaginary persons rather than them, and he would sit for long periods talking about money and workers who were not present. His symptoms worsened significantly after the death of his mother a month ago, when he had to be forced to maintain hygiene even during funeral rituals. Due to increasing severity, he was admitted to JRC, where he received treatment and is now stable and manageable. He continues follow‑up on an outpatient basis with antipsychotic medication and supportive therapies. His presentation, with chronic psychotic symptoms, poor self‑care, social and occupational dysfunction, and exacerbation after stress, is suggestive of a schizophrenia spectrum disorder.
